Henry Ford Hamtramck Medical Center will pilot an innovative community health worker (CHW) model to improve its existing behavioral health integration and collaborative care model. Henry Ford will develop an expanded model of care within the integrated care team, allowing the whole team to work to improve monitoring and follow-up care for vulnerable senior patients and reduce costs. The project will test reimbursement for CHWs as part of the care team, using collaborative care codes.
