Past Grants

To strengthen behavioral health care access and capacity, Sunrise PACE will hire a behavioral health nurse practitioner and social worker to provide in-person psychiatric services, including medication management and counseling, to underserved older adults. Using a mix of direct care expansion, staff training, and continued collaboration with local mental health agencies, the project addresses system-wide capacity gaps by offering integrated, community-informed care that empowers caregivers and improves health outcomes for high-need participants.
