Spectrum Health Foundation aims to significantly reduce racial disparities in birth outcomes and improve maternal-child health among communities of color. Strong Beginnings is a partnership of agencies and community organizations and will offer enhanced rental and inter-conception services to high-risk pregnant women and infants up to 24 months of age. Community health workers will connect women to other services, provide case management services, and conduct home visits to offer social support, care coordination, education, and referrals to wrap-around services. A mental health coordinator will facilitate therapeutic support groups and provide crisis management and individual counseling.
