Past Grants

The Return Home Safe program is a collaboration among community-based organizations to address home accessibility issues and daily activity support to at-risk citizens. Habitat for Humanity Detroit will gather data on improved in-home recovery, home safety, reduced emergency department usage, and reduced hospital readmissions to develop a sustainable model and demonstrate the return on investment for in-home modifications for long-term health outcomes.
