Past Grants

The Peer Recovery Support System will use a multi-sector approach supporting timely transitions to care between hospitals, treatment providers, and community settings while addressing the upstream social determinant of health needs of patients. The project will strengthen the business case for a sustainable peer recovery service delivery and payment model for addictions beyond opioid use disorder.
