The North Central Community Health Innovation Region (NCCHIR) will contract with a consultant to provide technical assistance and training to improve implementation of the Collective Impact model. The NCCHIR was established to work to address social and economic needs that impact health. Specifically, this funding will support the collaborative to conduct and analyze a collective impact survey which will guide the transformative change process and serve as a benchmark for outcome measurement, conduct interviews to identify barriers to collaboration, and provide a workshop to address the results of the survey. This funding will also support the NCCHIR in building a unified system of coordinated referrals, development of a plan to encourage more resident voice in the work, development of funding strategies to support their services and operations and expand the steering committee to include multi-sector regional community-based agencies.
