Past Grants

This project will create a collaborative network of local health departments, Area Agencies on Aging, and local Community Mental Health Authorities to address health-related needs of adults 60 and older and develop a coordinated system by which the local agencies work together to better support patients and integrate services. They will specifically target community health centers in areas of the state that serve large populations of Medicare patients, starting with Northern Michigan. This initiative would also help advance work the Health Fund has supported with Age-Friendly Public Health efforts.
