Past Grants

This collaborative pilot project between A Brighter Way (ABW) and Packard Health will address a gap in integrated health care for formerly incarcerated individuals in Washtenaw County, engaging returned citizens to improve health outcomes and quality of life. Peer navigators from ABW will receive health coaching from Packard Health, complementing their role as mentors, while participants receive support, addressing their unique needs.
