Past Grants

Through this grant, JFS aims to enhance their CHW capacity by training and certifying four CHWs, focusing on Latino youth and families, adults with complex health needs exacerbated by social determinants of health, and vulnerable older adults, with the goal of extending reach, improving health outcomes, and facilitating Medicaid reimbursement for coordinated services to ensure sustainable support for the community.
