AHA will replicate a project model in Southeast and Western Michigan, in partnership with healthcare organizations, community-based organizations, and community stakeholders to support organizations to develop interventions based on three strategic pillars: Increasing Access to prevention, early detection, and treatment, Activating communities to address health disparities, and Advocating for sustained systems change. AHA will support and increase the capacity of 10 participating community organizations to address their community-led efforts with tools, resources, and models
