The Avenue Family Network, Inc. (“The Avenue”) intends to implement CADCare, a case management software, which will allow their team to better track falls, nutritious meals served, and the psychosocial well-being of participants in its adult day programs. CADCare is built specifically for adult day programs and offers comprehensive assessment tools and the ability to securely manage participant records electronically.
