Integrating behavioral health care into primary care presents a crucial opportunity to closing access gaps for Michigan residents. The Collaborative Care Model (CoCM) is a financially sustainable, integrated behavioral health model that enables a primary care provider, psychiatric consultant, and behavioral health care manager to collaboratively support the patient and their family in the primary care setting.

In recent years, MDHHS, Blue Cross Blue Shield of Michigan (BCBSM), the Health Fund and other philanthropic foundations have invested in expanding the model’s adoption Michigan.
