The Chaldean Community Foundation (CCF) is requesting grant funding to launch its health and wellness fitness program for people with intellectual and developmental disabilities (I/DD). The goals of the program are to improve the health and wellness of 50 unduplicated individuals with intellectual and developmental disabilities through a fitness program altered to meet special needs. CCF proposes to address the need for fitness and health programs tailored to individuals with I/DD and their caregivers utilizing a multiprong approach (i.e., primary care visit; nutritional education courses; fitness lessons and follow-up monitoring of health status).
