To improve coordination across health and social service providers in the Upper Peninsula, this project will build a secure data exchange that allows eight community-based organizations and 211 to share referrals in real time using their existing systems. Partners will test workflows with synthetic data, and new tools for training, onboarding, and project management will help ensure the system is responsive to the region’s rural context.
