MDHHS will facilitate a Community Information Exchange (CIE) project that leverages and simultaneously strengthens partnerships between healthcare and social care sectors to rectify community-based organizations (CBOs) capacity challenges in CIE participation. The project will use physician organizations (POs) as resources to develop a statewide framework, actionable standards, and CIE implementation guidelines to improve data sharing among CBOs and within diverse systems.
