Lansing School District is seeking support in addressing needs of school children and their families related to social determinants of health, wellbeing, and educational success by developing a coordinated system of care within the district to promote accessibility and utilization of health and social services. The district aims to address the fragmented service provisioning system that currently exists among stakeholders by using BH Works, a care coordination platform that allows school staff and community-based providers to track and share student care information. This platform is provided through the Michigan Department of Education for free to all school districts in Michigan.
