United Way of Southeastern Michigan will create a person-centered community information exchange to identify health and social service needs, including social determinants of health, and build a network of service providers to ensure those needs are met. This grant will support the initial demonstration project, where emergency food system partners, local information and referral services, and healthcare organizations will share information and connect with older adults in Wayne County who are experiencing food insecurity. Future development will connect the data exchange to other local or statewide systems, including the Michigan Health Information Network.
