The Resilient Communities Project (RCP)”, led by Starr Commonwealth, aims to address ongoing issues of poverty and inequitable access to educational, health and human services in Calhoun County, by piloting a proven poverty-alleviation model called Transition to Success (TTS). With a strong focus on the social determinants of health and their impacts on children and families, Starr and its community service partners will help address poverty as a “medical” condition and integrate community health workers to enhance care coordination. This project seeks to create a sustainable framework for improving community health and well-being, while prioritizing the voices of those most affected in decision-making processes.
