In Luce County, where nearly half of households live in poverty and rates of serious chronic illness are high, this initiative will expand access to palliative care for residents. Community members will receive evidence-based training in goal-oriented care, an approach that helps patients and families make informed decisions based on their values and priorities, and a Community Advisory Board including patients and caregivers will guide implementation; a digital guidebook will be developed to support replication in other rural communities.
