Past Grants

Greater Midland Community Centers, in collaboration with MidMichigan Health, will scale an initiative to improve the health of individuals at risk of long-term chronic disease by connecting the medical community through The Physician Referred Wellness Advocate initiative. It combines best practices from health care – including physician-guided referrals – with best practices from wellness, one-on-one coaching, social supports, and built-in accountability to drive consistent participation in physical activity and healthy lifestyles with lasting effect.
