Past Grants

This project will improve medication management among older adults with diabetes and hypertension by virtually embedding pharmacists into patient care teams and enhancing patient-pharmacist-physician communication. As telehealth visits increase, this program will increase care coordination among physicians and pharmacists to reduce medication interactions and side effects and promote better health outcomes.
