To address the concerning trend of sleep-related infant deaths in Macomb County, this project focuses on reducing infant mortality rates through community health workers (CHWs) recruited from target communities, who will provide holistic education, mental health support, and client-centered care plans particularly for individuals experiencing a lack of prenatal care. The neighborhood-specific CHWs supported by a professional development center will focus on high-risk birthing parent-baby dyads and provide connections to necessary follow-up care in collaboration with birthing center social workers.
