Past Grants

To improve service delivery for older adults, this project will modernize data systems and train case managers in new tools and workflows. These upgrades will help staff coordinate care more effectively, reduce delays in services, improve care coordination, respond more quickly to older adults in crisis and measure outcomes across programs.
