Past Grants

This grant will directly address the social determinants of health through obtained data to assist with future policy development to address the root causes of poor health in communities. It will implement the Transition to Success™ (TTS) model to improve the health outcomes and economic status of the targeted populations by directly addressing the root causes of poverty with uniform protocols, analytics and methodologies that hold the delivery system accountable.
