This project seeks to reduce social isolation and improve care to LGBT older adults by implementing systemic changes involving policies, procedures, and training within Area Agencies on Aging in Michigan and Michigan 2-1-1. It replicates a successful 2014 Detroit-area pilot project, expanding it to three other regions of the state and upgrading it by solving training sustainability issues. This project will produce free online training modules and a toolkit that are available to all Michigan Area Agency on Aging staff.
