Past Grants

This project, in partnership with TBD Solutions, will help advance the IT readiness of the aging network by procuring a coordinated IT system that can help streamline services across providers – including healthcare partners and social service organizations – to create individualized care plans across multiple funding streams and share data for care coordination purposes with partners who share the same clients. These systems will help inform decision-making to improve clinical and non-clinical outcomes for clients.
