GenesisHOPE will establish a community-based “hub coordination” response system to address social determinants of health, manage chronic disease, and increase access to social and health services in the Greater Villages community in Detroit which includes the Islandview, West Village, Indian Village, and East Village Neighborhoods. The collaboration is based on the Community-Clinical Linkages for the Prevention and Control of Chronic Disease framework and will leverage existing relationships with faith-based organizations and other community organizations to increase awareness of available services and support.
The collaborative will focus on food as medicine programs, peer-to-peer learning, evidence-based nutrition education, and collect and share key performance indicators within and outside of the community to advocate for systems and policy change. They intend to hire an external evaluator to develop an evaluation plan and a detailed report on the progress of the collaborative, program participation, and gather community input.
