Macomb Children’s Healthcare Access Program (MCHAP) will implement the Foster CARE (Coordinating Access to Resources and Education) program to serve foster youth in Macomb County and increase the youths’ access to healthcare. MCHAP’s Foster CARE pilot program will utilize the medical home model to improve healthcare access for youth in foster care. MCHAP will hire one Community Health Worker (CHW), to work alongside the existing CHW at MCHAP, who will ensure that all 100 participating youth clients receive their 30-day comprehensive health assessment (CHA).
