This initiative will address the negative health effects of poor blood pressure measurement, diagnosis, and treatment at 10 Federally Qualified Health Centers (FQHCs), and improve hypertension control rates within the FQHC practices by implementing an enhanced version of Target:BP, a program led by the American Heart Association and American Medical Association. This enhanced version of Target:BP will build upon previously funded initiatives and link provider care with online and digital tools and resources and support from partner centers.
