Past Grants

Packard Health, a federally qualified health center serving Ann Arbor and Ypsilanti, will create a roadmap for the incubation and implementation of new programming to accompany renovations of their space. As part of this request, they will ensure patient voices are heard as part of the planning process and that new programming is rooted in their realities and needs. Over time, Packard Health anticipates this will lead to an integrated care model that includes evidence-based wellness and prevention programming.
