Michigan State University will design the Caring for Patients with Chronic Conditions residency curriculum (CPCC) and deliver the curriculum for all MSU internal medicine residents. Residents will work with community partners to coordinate medical and non-medical services when caring for patients, resulting in holistic, cost-effective care plans that embrace patient-centered clinical practices and can be assessed using evidence-based health outcome measures.
