This project will create a telehealth and wellness hub at a senior center in Houghton Lake, offering telehealth access, digital literacy, dementia support and chronic disease management. It will serve adults aged 60 years or older and formal or informal caregivers with a focus on Roscommon County and surrounding rural communities. It will also offer training and education for providers in dementia diagnosis, care planning, and support strategies.
