Funding will enable the expansion of the Mid-Michigan Community Health Access Program (CHAP) to reach residents in Michigan’s 8th Congressional district (Flint, Saginaw, and Bay City). The project aims to identify individuals with unmet social needs, provide coordinated care management and community linkages, and leverage community health workers to address diverse social determinants of health, ultimately improving health outcomes and connecting residents to vital services. This grant will allow the program to be tested in a rural area while Medicaid redetermination is happening across the state.
