The purpose of this project is to reduce barriers and improve timely access to quality care for Medicaid-insured women of color by integrating a hypertension disorders of pregnancy (HDP) intervention into community home visiting and clinical sites to address inequities in maternal mortality and morbidity. Michigan State University will create a standardized training protocol and coordinate implementation in a range of home visitation programs and clinical settings. The lessons learned will be replicable across the state and will be sustained through community health worker training.
