This project will physically co-locate the PACE program in a dedicated wing of assisted living (AL) communities, using their own staff and creating an on-site “Place for PACE.” They will partner with AL facilities in Warren and Troy to establish and test the model in three ways: 1) to offer wraparound services for those with declining health and mild memory decline, 2) to focus on behavioral health and dementia; and 3) to provide a regional “hub” for rehab participants. The initiative also involves a unique partnership with Detroit Cristo Rey High School, engaging students in a work study program to enhance their skills and provide support to PACE participants, enriching the overall care experience.
