Past Grants

To improve early identification and integrated care for youth in foster care, this project will deploy a mobile, multidisciplinary team to conduct comprehensive physical and behavioral health screenings within 72 hours of placement. Led by GVSU, the Enhanced Integrated Care Model connects youth to timely interventions and coordinated follow up across medical, behavioral, and social services—reducing fragmentation and addressing health inequities in Kent County’s foster care system.
