This is a planning grant to create and sustain a local community information exchange, reducing care silos and increasing connections between community care systems. During the planning phase, the Michigan Center for Clinical Systems Improvement will establish a lead planning group and ad hoc work groups, design the community care model, identify potential technology solutions, and create a sustainability plan.
