The Area Agency on Aging 1-B (AAA 1-B) aims to reduce the complexity and stress of managing care for seniors or those with disabilities by enhancing its SameAddress dCare platform to include individual medical profiles and centralized information about care provided through the SameAddress program. dCare will include personalized content, so individuals and caregivers can better communicate about upcoming appointments and learn more about their care and conditions.
