Wellness Through In-Home Physical Activity Partnering

This initiative will create an easy-to-use in-home physical activity program for homebound older adults and their caregivers. Formal and informal caregivers will receive training on how to motivate and partner with the person in their care to implement the activity regimen. The program will primarily target participants with a history of falls and frequent hospitalizations. Partners in the model development will include local Board Certified Geriatric Clinical Specialist, Doctor of Physical Therapy, and NYMBL, an evidence-based program geared toward older adult learning strategies. Once the model is established, the program would be eligible for reimbursement through Older American Act funds and Michigan’s Medicaid Home Community Base Services (MI-Choice Waiver). This program is a direct result of input from clients, caregivers, and community partners and Region II’s advisory council, consisting of older adult community members and representatives from partner agencies, will provide feedback and guidance throughout the development and implementation process.
