Past Grants

The Alzheimer’s Association Michigan Chapter, along with partnering VA medical centers across Michigan, will work closely together to evaluate and improve policies and processes regarding dementia care at the medical centers and improve access to services for Veterans and their caregivers through the Alzheimer’s Association. The project aims to engage VA medical center leadership and clinicians to improve dementia detection, diagnosis, and disclosure, as well as enhance clinical capabilities around care planning and disease management. The project will also provide greater access to a variety of additional community-based care and support services for Veterans and their caregivers through the Alzheimer’s Association chapters across Michigan.
