Baldwin Family Health Center (BFHC), a federally qualified health center located in rural northwest Michigan, will serve 150 older adults with chronic conditions in Lake and Newaygo County with this telehealth initiative. The project will provide timely access to care through outreach workers, who will help provide primary care, behavioral health, and pharmacy services through virtual consults
