Project Thrive will improve the overall health and well-being for socioeconomically disadvantaged, medically underserved, high-risk adults by addressing the unmet mental health needs of this vulnerable population. Project Thrive will fully integrate behavioral health services in its existing person-centered, interdisciplinary team approach to primary care, using the Advancing Integrated Mental Health Solutions model from the University of Washington. This project will also train future nursing and health professionals through behavioral health clinical rotations at the Grand Valley State University Kirkof College of Nursing Family Health Center.
