Past Grants

The goal of this initiative is to develop a network of volunteer caregivers to combat isolation and help homebound individuals maintain their independence to age in place. This proposal will demonstrate the feasibility of cost-effective and scalable approaches to triple the number of volunteer caregivers in six communities, covering different parts of the state.
