This initiative is an older adult-focused pre- and post- surgical program to help improve outcomes and provide a care transition plan so hundreds of people can receive the follow-up care they need at home. The program will reduce complications and hospital readmissions and connect older adults and caregivers to other programs and services in the community to help aid in recovery.
