Past Grants

This project builds on a 2024 Healthy Aging grant to develop the Michigan Community Care Collaborative to establish and manage contractual agreements with health-based organizations and insurance companies. This grant will implement and deploy a comprehensive system to streamline processes such as provider contracting, credentialing, and data sharing, ensuring efficiency and sustainability.
