Cass Community Social Services will integrate an evidence-based standard of care in its free, volunteer-run community clinic. Transition to Success is a social care model, and in this adaptation, will train medical students and community volunteers to screen and assess patients for social determinants of health and poverty-driven social vulnerabilities.
