This program builds off the Primary Care at Home program and integrates access to palliative care services. Advanced practice providers will be embedded in home-based primary care teams to deliver palliative care to older adults. The program will serve 200 patients annually and partner with Harbor Hospice to ensure older adults have access to palliative care services.
