This project will develop and expand an innovative and cost effective model for chronic disease prevention by connecting primary care and treatment of chronic disease with evidence-based prevention practices. The project aims to increase access to healthy foods among vulnerable populations, particularly in rural communities; educate residents on healthy food choices and physical activity; and develop a sustainable model for integrating community and healthcare resources to support residents who are living with or at-risk for chronic diseases.
