The program will enhance and expand Matrix Human Service’s Center of Senior Citizens to address health disparities among low-income senior citizens by increasing access to healthy, fresh food and health screenings. The program will provide case management and care coordination to participants to ensure program retention, help with transportation and encourage healthy lifestyle habits.
