The Little Victors League (LVL) provides a comprehensive care coordination program dedicated to infants born with medical complexities and their families, enabling them to thrive and live their best lives. This project builds off the success of the Partners for Children project previously supported by the Health Fund and expands infants. An emphasis of this program is to coordinate care that aligns with key social determinants of health to support economic, community, and family health. LVL care coordinators are nurses and social workers dedicated to LVL infants.
